Dosage and direction

The initial dose of Orlijohn must be individualized based on the patient’s prior opioid exposure, medical status, and analgesic needs. For opioid-naïve patients, treatment usually begins with the 10mg tablet every 12 hours. Dosage titration should occur in increments of 10mg every 3 to 5 days, as tolerated, to achieve adequate pain control. Tablets must be swallowed whole and must not be crushed, chewed, or dissolved, as this can lead to rapid release and potential overdose. Administration with food is optional but consistent intake (with or without meals) is advised to maintain uniform absorption.

Precautions

Patients should be cautioned about the risks of respiratory depression, especially during initiation and following dosage increases. Missed dose

If a dose is missed, it should be taken as soon as possible. However, if it is almost time for the next dose, the missed dose should be skipped and the regular dosing schedule resumed. Doubling the dose is not recommended.

Overdose

Symptoms of overdose may include respiratory depression, extreme somnolence, muscle flaccidity, cold/clammy skin, and bradycardia. In case of suspected overdose, seek emergency medical attention immediately. Naloxone may be used as an antidote under medical supervision.

Storage

Store at room temperature (20–25°C) in a dry place, away from light and moisture. Keep out of reach of children and pets. Dispose of unused medication properly via a drug take-back program.
