Bronchodilators play a key role in the complex treatment of diseases of the respiratory system caused by the development of obstruction. Currently, a number of drugs aimed at relieving bronchospasm are used in medical practice. One of the most effective is the following drug.
This drug is included in the group of anti-asthmatic medicines used in the form of inhalations.
The main active components of Combimist-L Inhaler are Ipratropium Bromide and Salbutamol. Ipratropium Bromide is a quaternary ammonium compound that has anticholinergic properties. It is a competitive antagonist of a neurotransmitter such as acetylcholine.
Ipratropium Bromide produces a blocking effect on some neurotransmitters, which causes blocking the transmission of nerve impulses.
This component is also capable of blocking smooth muscle receptors located in the tracheobronchial tree.
Due to its anticholinergic properties, the drug prevents an increase in the concentration of certain substances in the cells, which causes its broncholytic effect, as well as preventive opportunities for the development of bronchospasm. Also, its use leads to a decrease in the number of secretions from the bronchi and sinuses.
Patients suffering from bronchospasm observe a marked improvement in health and lung function after the first quarter of an hour after application.
Salbutamol is a sympathomimetic that can stimulate beta-two-adrenergic receptors. This component leads to the relaxation of smooth muscles in the bronchi, blocking inflammatory mediators, increasing mucociliary clearance.
Indications for using Combimist-L Inhaler
Combimist-L Inhaler is used for long-term treatment of bronchospasm caused by:
- chronic obstructive disorders;
- chronic bronchial asthma.
Contraindications to using Combimist-L Inhaler
The drug in question is contraindicated in cases where the patient has a pronounced hypersensitivity (allergy) to the main or to one of the auxiliary components.
It is also contraindicated for use in:
- hypertrophic obstructive cardiomyopathy;
- tachyarrhythmia.
In pediatrics, it can be used from the age of 12.
Use during pregnancy and lactation
Combimist-L Inhaler is contraindicated for pregnant women in the first trimester of pregnancy.
If it is necessary to take the drug during breastfeeding, lactation is suspended.
Dosage and administration
The drug Combimist-L Inhaler is taken by inhaling a dose from an inhaler.
To stop an acute attack of bronchial asthma or shortness of breath, adult children over 12 years of age should take 1 ampoule 3-4 times a day, in the form of inhalations.
In children, dosed aerosol should be used only on the recommendation of a doctor and under the supervision of adults.
The duration of treatment depends on the severity of the disease and is determined individually.
If there is no therapeutic effect from the use of the drug or if the patient's condition worsens, or if there is a noticeable decrease in the effectiveness of the prescribed dose, the patient should consult a doctor for additional measures.
Application features of Combimist-L Inhaler
It should be explained to the patient that in the case of acute shortness of breath, which quickly worsens, or a clear decrease in the reaction to the drug, it is necessary to consult a doctor.
After using the drug, hypersensitivity reactions of an immediate-type are possible, rarely manifested by urticaria, angioedema, rash, bronchospasm, oropharyngeal edema, and anaphylaxis.
There have also been occasional reports of a number of ophthalmic complications when ipratropium bromide, in the form of an aerosol, alone or in combination with a beta-2-adrenergic agonist, accidentally got into the eyes. Therefore, patients should be instructed on how to use Combimist-L Inhaler with a nebulizer properly and warned that the solution or its drops should not get into the eyes when sprayed. To avoid accidental contact with the drug in the eyes, it is better to apply the solution using a mouthpiece rather than a face mask.
Such ophthalmic complications may include acute angle-closure glaucoma, mydriasis, blurred vision, increased intraocular pressure, eye pain, and narrow-angle glaucoma. Patients at risk of glaucoma should be warned about the need for eye protection. In such patients, anti-glaucoma therapy is effective for the prevention of acute narrow-angle glaucoma.
Common use
Combimist L Inhaler is composed out of the following active ingredients:
Levosalbutamol – 50 mg and Ipratropium bromide – 20 mg

Dosage and direction
Combimist L Inhaler is available only in the form of a nasal spray but must be used through the mouth, inhaled.
The inhalation is recommended up to 3 times a day with a maximum dosage of 42 mcg per nostril ( ~two sprays)

Contraindications
Ipratropium bromide is not to be used on patients with knows hypersensitivity to atropine and all of its derivates.
Using Ipratropium bromide in relation with curing the bronchospasm in acute COPDS exacerbation if not firmly studied. Other drugs with faster effect might be used as an alternative.
Combining Ipratropium bromide and beta agonists is not known as a better effective combination than when using the drug alone when it comes to curing the bronchospasm in association with the acute COPD exacerbation.
Some rapid hypersensitivity reaction might come after taking Ipratropium bromide, thing which is proved by rare cases of urticaria, angioedema, rash, oropharyngeal edema and bronchospasm.
Possible side effect
Is well known that levosalbutamol is well tolerated. General side effects may include: Faster heart rate, muscle pain and dysfunction of the gastric system.
The following points are part of a list which includes some side effect that may occur when using Combimist L Inhaler.
The list is not yet completed but these available side effects are not always to take place. Some might be very rare although serious. Get in touch with your doctor in case of any signs of such side effects, especially in the cases when it gets worse.
- Severe hypokalemia
- Tremors especially of hands
- Vomiting
- Diarrhea
- Tiredness
- Urge to vomit
- Dizziness
- Muscle cramps

Drug interraction
Ipratropium bromide is known to be a non toxic and very efficient solution when used in combination with beta adrenergic bronchodilators. Ipratropium bromide is also in use with other pulmonary medications, such as methylxanthines and corticosteroids, without any adverse drug interactions.
Missed dose
Always take the missed dose when you remember. If that time is closer to the next dose than the forgotten one, skip the missed dose and proceed to the next one. Never take 2 doses in the same time and never take extra volume of this drug. Get in touch with your doctor if you feel insecure about your actions.
Overdose
In case you feel an overdose has occurred, always call your local poison control center or Hospital, first thing. The following are signs of possible over dose reaction:
wheezing;
- chest tightness;
- fever;
- itching;
- bad cough;
- blue or gray skin color;
- seizures;
- or swelling of face, lips, tongue, or throat;
- trouble breathing;
- sudden change in eyesight, eye pain, or irritation;
- any rash.
Storage
Keep Combimist L Inhaler out of the reach of children and pets, in dry places and at temperatures not higher than 25 C/77 F. The product may be used at temperatures from 15 C to 30 C (59 F to 86F) but for short periods of time. Keep the product safe from extreme heat and moisture.
