Xenical (orlistat) is a prescription-only lipase inhibitor designed for the long-term management of obesity in conjunction with a reduced-calorie diet. It functions by blocking the absorption of approximately one-third of the fat from the food you consume, providing a scientifically-backed, non-systemic approach to weight loss. This medication is indicated for patients with an initial body mass index (BMI) of 30 kg/m² or greater, or 27 kg/m² or greater in the presence of other risk factors such as hypertension, type 2 diabetes, or dyslipidaemia. Dosage and direction
The recommended dosage of Xenical is one 120 mg capsule taken orally three times daily with each main meal containing fat (during the meal or up to one hour after the meal). The dose can be omitted if a meal is missed or if the meal contains no fat. Distribution of fat intake equally across the three main meals is advised to manage gastrointestinal effects. Supplementation with a multivitamin containing fat-soluble vitamins (A, D, E, and K) is recommended, to be taken at least 2 hours before or after Xenical administration.
Precautions
Patients should be advised to follow a nutritionally balanced, reduced-calorie diet. A diet with more than 30% of calories from fat may increase the likelihood of gastrointestinal adverse reactions. Use with caution in patients with a history of hyperoxaluria or calcium oxalate nephrolithiasis. Monitor patients for any unusual bruising or bleeding, which could indicate a reduction in vitamin K absorption. Not intended for use in paediatric populations. Periodic monitoring of weight loss and progress is essential.
Contraindications
Xenical is contraindicated in patients with chronic malabsorption syndrome or cholestasis. It must not be used in patients with known hypersensitivity to orlistat or any component of the formulation. It is contraindicated during pregnancy and breastfeeding. Use is not recommended in patients with anorexia nervosa or bulimia nervosa.
Possible side effect
The most common side effects are gastrointestinal in nature and are a direct consequence of the drug’s mechanism of action, resulting from undigested fat passing through the intestines. These include:
- Oily spotting from the rectum
- Flatus with discharge
- Faecal urgency
- Fatty/olly stool
- Oily evacuation
- Increased defecation
- Faecal incontinence These events are generally mild to moderate, occur early in treatment, and are often transient. Other side effects may include headache, upper respiratory infection, and abdominal pain/discomfort. Rare cases of hypersensitivity and severe liver injury have been reported.
Drug interaction
Orlistat may reduce the absorption of fat-soluble vitamins (A, D, E, K) and beta-carotene. It may also impair the absorption of concurrently administered lipophilic drugs. Administer the following medications at least 2 hours before or after Xenical, or at bedtime: cyclosporine, levothyroxine, warfarin and other anticoagulants, antiepileptic drugs (e.g., valproate, lamotrigine), and amiodarone. Monitor INR levels closely in patients on warfarin therapy. Orlistat can potentially reduce the efficacy of oral contraceptives; advise patients to use an alternative non-oral method of contraception.
Missed dose
If a dose is missed, resume the usual schedule with the next meal. Do not take a double dose to make up for a missed one. The dose should only be taken when a meal contains fat.
Overdose
There is no specific antidote for orlistat overdose. Single doses of 800 mg and multiple doses up to 400 mg three times daily for 15 days have been studied without significant adverse outcomes. Based on its minimal systemic absorption, no serious systemic toxic effects are expected. Medical attention should be sought, and treatment should be symptomatic and supportive.
Storage
Store Xenical capsules at room temperature between 15°C and 30°C (59°F and 86°F). Keep the bottle tightly closed and in the original container to protect from moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the bottle.
