Viagra (sildenafil citrate) is a prescription phosphodiesterase type 5 (PDE5) inhibitor, representing a cornerstone in the pharmacological management of erectile dysfunction (ED). Developed through rigorous clinical research, it facilitates the achievement and maintenance of erections sufficient for satisfactory sexual activity by enhancing blood flow to the penile corpora cavernosa upon sexual stimulation. Its mechanism of action is well-established, targeting the underlying physiological processes involved in erection, making it a first-line therapy option prescribed by healthcare professionals worldwide for treating this common medical condition.
Features
- Active Pharmaceutical Ingredient: Sildenafil citrate.
- Standard Dosage Strengths: Available in 25 mg, 50 mg, and 100 mg oral tablets.
- Mechanism of Action: Selective inhibitor of cGMP-specific phosphodiesterase type 5 (PDE5).
- Onset of Action: Typically within 30 to 60 minutes post-administration.
- Duration of Effect: The therapeutic window for efficacy generally lasts up to 4 hours, though individual response may vary.
- Administration: Orally ingested, with or without food (though high-fat meals may delay absorption).
Benefits
- Restores Physiologic Erection Function: Effectively facilitates the natural erectile response to sexual stimulation by inhibiting PDE5, thereby increasing concentrations of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum and promoting smooth muscle relaxation and vasodilation.
- High Efficacy Rates: Demonstrated significant improvement in erectile function across a broad spectrum of ED severities and etiologies (organic, psychogenic, and mixed) in large-scale, double-blind, placebo-controlled clinical trials.
- Improved Sexual Confidence and Relationship Satisfaction: Successful treatment of ED can lead to enhanced self-esteem, reduced performance anxiety, and improved overall quality of life for the patient and their partner.
- Well-Established Safety Profile: As one of the most extensively studied medications for ED, its safety and tolerability are well-documented in a vast population over decades of clinical use.
- Predictable Pharmacokinetics: Offers a reliable duration of action, allowing patients to plan sexual activity with a known and consistent therapeutic window.
Common use
Viagra is indicated for the treatment of erectile dysfunction, a condition characterized by the consistent or recurrent inability to achieve and/or maintain a penile erection sufficient for satisfactory sexual performance. It is used by adult males and is not indicated for use by women, children, or newborns. Its use is predicated on the presence of sexual stimulation; it does not cause an automatic erection.
Dosage and direction
The recommended starting dose for most patients is 50 mg taken approximately one hour before anticipated sexual activity. Based on efficacy and tolerability, the dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg. Dosing should be limited to a maximum of once per 24-hour period. The tablet should be swallowed whole with a glass of water. Administration with a high-fat meal may delay the rate of absorption, reducing the maximum plasma concentration and slowing the time to onset of effect.
Precautions
- Cardiovascular Status: Patients should have a cardiovascular evaluation prior to initiating any treatment for ED. Sexual activity carries a potential cardiac risk, especially in those with pre-existing cardiovascular disease.
- Priapism: Prolonged erection lasting more than 4 hours and priapism (a painful erection lasting more than 6 hours) have been reported infrequently. This condition requires immediate medical attention to prevent long-term penile tissue damage and permanent impotence.
- Vision and Hearing Changes: Non-arteritic anterior ischemic optic neuropathy (NAION), a cause of decreased vision or permanent vision loss, and sudden decrease or loss of hearing have been reported in temporal association with PDE5 inhibitor use. Patients should be advised to seek immediate medical attention if these symptoms occur.
- Underlying Causes of ED: Treatment with Viagra does not protect against sexually transmitted diseases. A thorough medical evaluation should be conducted to diagnose ED and determine potential underlying causes.
Contraindications
- Concomitant administration with any form of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate or dinitrate) or nitric oxide donors (e.g., amyl nitrite “poppers”) in any form, either regularly or intermittently. This combination can cause a profound and potentially life-threatening drop in blood pressure.
- Hypersensitivity to sildenafil or any of the tablet’s excipients.
- Concurrent use with guanylate cyclase stimulators, such as riociguat.
Possible side effect
Viagra is generally well-tolerated. The most common adverse reactions are largely dose-dependent and are mediated by the pharmacologic vasodilatory effects. They include:
- Headache
- Flushing
- Dyspepsia (indigestion)
- Nasal congestion
- Dizziness
- Visual disturbances (e.g., color tinge to vision, blurred vision, increased light sensitivity)
- Back pain and myalgia Less common side effects include hypotension, syncope, and palpitations.
Drug interaction
- Nitrates: Absolute Contraindication. Concomitant use can lead to severe hypotension, syncope, myocardial infarction, or stroke.
- Alpha-adrenergic blockers: (e.g., doxazosin, tamsulosin). Concomitant use can potentiate the blood-pressure-lowering effects. Blood pressure should be stabilized prior to initiating Viagra, and a starting dose of 25 mg should be considered.
- Other Antihypertensives: Additive effects on blood pressure are possible when co-administered with other vasodilators or antihypertensive medications.
- CYP3A4 Inhibitors: Strong inhibitors of the cytochrome P450 3A4 enzyme (e.g., ketoconazole, itraconazole, ritonavir, clarithromycin) can significantly increase sildenafil plasma concentrations. A maximum dose of 25 mg within a 48-hour period is recommended with strong CYP3A4 inhibitors.
- CYP3A4 Inducers: (e.g., rifampin) may decrease sildenafil plasma concentrations, potentially reducing efficacy.
Missed dose
Viagra is not intended for chronic daily dosing but is taken on an as-needed basis. Therefore, the concept of a “missed dose” does not apply. Do not take an extra dose to make up for a missed opportunity for sexual activity. Adhere to the maximum dosing frequency of once per day.
Overdose
In cases of overdose, the expected clinical presentation would be an exaggeration of the known dose-dependent adverse effects, including severe hypotension, syncope, and prolonged erection. Standard supportive measures should be employed as required. Given its high protein binding, sildenafil is not expected to be dialyzable. Management of priapism should involve immediate urological consultation.
Storage
Store at room temperature between 20°C to 25°C (68°F to 77°F), with excursions permitted between 15°C and 30°C (59°F and 86°F). Keep the medication in its original container to protect from light and moisture. Keep out of reach of children and pets.
