Common use

Dosage and direction
The recommended dose is one 10mg tablet taken orally with water at the onset of a migraine attack. A second dose may be taken if symptoms persist or return after an initial response, but not within a 24-hour period. The total daily dose should not exceed 20mg. The tablet should not be split or crushed. Efficacy may be reduced if taken during the aura phase before the headache begins.
Precautions
Patients should be advised not to use Varnitrip if they have unrecognized cardiovascular disease. A cardiovascular evaluation is recommended for patients with multiple risk factors before initiating treatment. Use with caution in patients with hepatic impairment, as metabolism may be affected. This medication may cause drowsiness or dizziness; patients should be cautioned about operating machinery or driving until they know how it affects them.
Contraindications
Varnitrip is contraindicated in patients with a history of ischemic heart disease, coronary artery vasospasm, uncontrolled hypertension, cerebrovascular syndromes, or peripheral vascular disease. It is contraindicated in patients with severe hepatic impairment. Concomitant use within 24 hours of ergotamine-containing medications or other 5-HT1 agonists is contraindicated. Serious but rare side effects include coronary vasospasm, arrhythmias, and myocardial infarction. Patients should be instructed to seek immediate medical attention for any symptoms suggestive of a serious cardiac event.
Drug interaction
Varnitrip must not be used concomitantly with MAO-A inhibitors or within two weeks of discontinuing such therapy. Significant interactions may occur with other serotonergic drugs (e.g., SSRIs, SNRIs, tricyclic antidepressants), potentially increasing the risk of serotonin syndrome. Concurrent use with other vasoconstrictors should be avoided. Potent CYP3A4 inhibitors may increase plasma concentrations of Varnitrip.
Missed dose
As Varnitrip is used on an as-needed basis for acute attacks, the concept of a “missed dose” does not apply. It should only be taken at the onset of migraine symptoms. Do not take a dose to “make up” for a previous unattacked migraine.
Overdose
Symptoms of overdose may include hypertension, more pronounced versions of the common side effects, and cardiovascular events. In cases of suspected overdose, supportive measures should be instituted, including continuous ECG monitoring for at least 24 hours and symptomatic treatment. There is no specific antidote. The patient should be transported to a hospital emergency department immediately.
Storage
Store at room temperature (20-25°C or 68-77°F) in the original blister package to protect from light and moisture. Excursions are permitted between 15-30°C (59-86°F). Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
