Secnidazole is a second-generation nitroimidazole antimicrobial agent specifically designed to treat a range of anaerobic bacterial and protozoal infections. It is distinguished by its prolonged half-life, which allows for single-dose or short-course regimens, enhancing patient adherence and reducing the likelihood of treatment failure. This makes it a valuable option in clinical practice for managing conditions like bacterial vaginosis, amoebiasis, giardiasis, and trichomoniasis. Its mechanism involves the disruption of DNA synthesis in susceptible microorganisms, leading to cell death.

Features

  • Active ingredient: Secnidazole
  • Drug class: Nitroimidazole antibiotic
  • Available forms: Oral granules (2g sachets), tablets
  • Administration: Typically single-dose or short-course
  • Half-life: Approximately 17–29 hours
  • Bioavailability: High oral absorption

Benefits

  • Single-dose convenience improves patient compliance and reduces missed doses.
  • Broad-spectrum activity against anaerobic bacteria and protozoa.
  • Effective resolution of symptoms such as discharge, odor, and abdominal discomfort.
  • Lower risk of resistance development compared to some shorter-acting alternatives.

Common use

Secnidazole is commonly prescribed for bacterial vaginosis, amoebiasis (intestinal and hepatic), giardiasis, and trichomoniasis. It is also used for prophylaxis in colorectal surgery to prevent postoperative anaerobic infections.

Dosage and direction

For bacterial vaginosis: a single 2g dose orally. For amoebiasis: 2g daily for 3 days in adults; pediatric dosing based on weight. For trichomoniasis and giardiasis: a single 2g dose in adults. Always take with food to minimize gastrointestinal upset. The granules should be sprinkled on applesauce, yogurt, or pudding and consumed immediately without chewing.

Precautions

Avoid alcohol consumption during treatment and for at least 72 hours after completion due to the risk of a disulfiram-like reaction. Use with caution in patients with central or peripheral nervous system disorders. Not recommended during the first trimester of pregnancy unless clearly needed.

Contraindications

Hypersensitivity to secnidazole, other nitroimidazoles, or any component of the formulation. History of blood dyscrasias. Active organic neurological disorders.

Possible side effect

Common side effects include nausea, vomiting, metallic taste, headache, and dizziness. Less frequently, diarrhea, abdominal pain, or allergic reactions such as rash may occur. Neutropenia and peripheral neuropathy are rare but serious adverse effects.

Drug interaction

Concomitant use with disulfiram, alcohol, or alcohol-containing products is contraindicated. May potentiate the effect of warfarin, requiring INR monitoring. Caution with drugs that induce CYP450 enzymes.

Missed dose

If a dose is missed and it is close to the time of the next dose, skip the missed dose. Do not double the dose. For single-dose regimens, take as soon as remembered unless it is near the next scheduled dose.

Overdose

Symptoms may include nausea, vomiting, ataxia, and seizures. There is no specific antidote; treatment is supportive and symptomatic. Gastric lavage may be considered if ingestion was recent.

Storage

Store at room temperature (15–30°C) in a dry place. Keep the sachet or tablet in the original packaging. Keep out of reach of children and pets.
