Common use

Nasonex is primarily indicated for the treatment of seasonal and perennial allergic rhinitis symptoms. It is also approved for the prophylaxis of seasonal allergic rhinitis symptoms when initiated 2–4 weeks prior to the anticipated start of the pollen season. Additionally, it is used for the treatment of nasal polyps in adults, both as a monotherapy and as an adjunct to surgery.

Dosage and direction

For allergic rhinitis in adults and adolescents 12 years and older: 2 sprays in each nostril once daily (total 200 mcg/day). For children 2–11 years: 1 spray in each nostril once daily (total 100 mcg/day). For nasal polyps in adults: 2 sprays in each nostril twice daily (total 400 mcg/day). Shake well before use. Prime the pump before first use or if not used for 14 days or more. Blow nose gently before administration. Tilt head slightly forward and insert tip into nostril, pointing away from the nasal septum. Breathe in gently while spraying.

Precautions

Avoid use in patients with recent nasal septum ulcers, nasal surgery, or nasal trauma until fully healed. Use with caution in patients with tuberculosis, untreated fungal, bacterial, or viral infections, or ocular herpes simplex. Monitor for signs of adrenal suppression if used long-term or at high doses. Nasal corticosteroids may slow growth in children; regularly monitor pediatric patients. Avoid exposure to chickenpox or measles if not previously immunized.

Contraindications

Hypersensitivity to mometasone furoate or any component of the formulation. Not for use in the presence of untreated localized infection involving the nasal mucosa.

Possible side effects

Common side effects (≥1%) include headache, nasal irritation, pharyngitis, epistaxis, and cough. Less frequently reported effects include nasal burning, ulceration, and perforation of the nasal septum. Systemic effects such as adrenal suppression, glaucoma, cataracts, and growth suppression may occur with prolonged use or high doses. Immediate hypersensitivity reactions, including urticaria, angioedema, and bronchospasm, have been reported rarely.

Drug interaction

No formal drug interaction studies have been conducted with Nasonex. However, caution is advised with concomitant use of strong CYP3A4 inhibitors (e.g., ketoconazole), which may increase systemic exposure to mometasone. Use with other nasal corticosteroids may increase the risk of hypothalamic-pituitary-adrenal (HPA) axis suppression.

Missed dose

If a dose is missed, administer as soon as remembered. If it is near the time of the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose to catch up.

Overdose

Acute overdose is unlikely due to low systemic bioavailability. If excessive doses are administered chronically, systemic glucocorticoid effects such as hypercorticism and adrenal suppression may occur. Treatment should be symptomatic and supportive. There is no specific antidote.

Storage

Store at room temperature (15–30°C or 59–86°F). Do not freeze. Keep the bottle tightly closed and stored with the dust cap in place. Discard after 120 actuations following initial priming, as the amount of medication per spray may decrease substantially thereafter.
