Micardis (telmisartan) is an angiotensin II receptor blocker (ARB) indicated for the treatment of hypertension in adults. It works by blocking the action of certain natural substances that tighten blood vessels, allowing blood to flow more smoothly and the heart to pump more efficiently. This medication may also be used to reduce the risk of myocardial infarction, stroke, or death from cardiovascular causes in patients 55 years of age or older at high risk of developing major cardiovascular events who are unable to take ACE inhibitors. Clinical studies have demonstrated its efficacy in providing 24-hour blood pressure control with a single daily dose. Proper use under medical supervision can significantly contribute to long-term cardiovascular risk reduction.
Features
- Active ingredient: Telmisartan
- Medication class: Angiotensin II receptor blocker (ARB)
- Available in tablet strengths: 20 mg, 40 mg, and 80 mg
- Once-daily dosing regimen
- 24-hour blood pressure control
- Not a controlled substance
- Prescription-only medication
Benefits
- Provides consistent 24-hour blood pressure control with single daily dosing
- Reduces the risk of cardiovascular events in high-risk patients
- Demonstrates excellent tolerability profile with low incidence of side effects
- Shows beneficial metabolic effects including potential improvement in insulin sensitivity
- May offer organ-protective effects beyond blood pressure lowering
- Maintains efficacy throughout the dosing interval without trough effect
Common use
Micardis is primarily prescribed for the management of hypertension (high blood pressure) in adult patients. It may be used alone or in combination with other antihypertensive agents. Additionally, Micardis is indicated to reduce cardiovascular morbidity in patients aged 55 years or older at high risk of developing major cardiovascular events who are unable to take ACE inhibitors. This includes patients with documented coronary artery disease, peripheral arterial disease, previous stroke, or transient ischemic attack, or patients with diabetes mellitus with evidence of target organ damage. The medication is not recommended for use in pediatric patients under 18 years of age.
Dosage and direction
The recommended initial dose of Micardis is 40 mg once daily. Depending on blood pressure response, the dosage may be increased to 80 mg once daily. In patients with mild to moderate hepatic impairment, the daily dose should not exceed 40 mg. For patients with renal impairment, including those on hemodialysis, no initial dosage adjustment is necessary. Micardis may be administered with or without food. Tablets should be swallowed whole with a sufficient amount of fluid (e.g., one glass of water). The antihypertensive effect is substantially present within two weeks and maximal reduction is generally attained after four weeks of treatment.
Precautions
Before taking Micardis, inform your healthcare provider about any history of kidney disease, liver disease, heart problems, or electrolyte imbalances. Patients should avoid becoming pregnant while taking Micardis, as the medication can cause injury or death to the developing fetus. Regular monitoring of blood pressure, renal function, and serum potassium levels is recommended, especially during initial therapy and after dosage adjustments. Use with caution in patients with bilateral renal artery stenosis or stenosis to a solitary kidney. Patients should be advised against using potassium supplements or salt substitutes containing potassium without medical supervision. Caution is advised when administering to elderly patients, particularly those with impaired renal function.
Contraindications
Micardis is contraindicated in patients with known hypersensitivity to telmisartan or any component of the formulation. Do not co-administer with aliskiren in patients with diabetes. Contraindicated during the second and third trimesters of pregnancy due to the risk of fetal injury and death. Avoid use in patients with severe hepatic impairment (Child-Pugh class C). Not recommended for patients with severe renal impairment or end-stage renal disease requiring dialysis. Concomitant use with ACE inhibitors is generally not recommended due to increased risk of adverse events including hypotension, hyperkalemia, and renal impairment.
Possible side effects
Common side effects (≥1% and <10%) include upper respiratory tract infection, back pain, sinusitis, diarrhea, and pharyngitis. Less common side effects (≥0.1% and <1%) may include dizziness, headache, fatigue, indigestion, myalgia, and increased sweating. Rare but serious adverse reactions include angioedema, renal impairment, hypotension in volume-depleted patients, and hyperkalemia. Laboratory abnormalities may include increased blood creatinine levels, elevated liver enzymes, and hyperkalemia. Patients should seek immediate medical attention if they experience symptoms of allergic reactions, significant dizziness, fainting, or symptoms suggesting kidney problems.
Drug interaction
Micardis may interact with several medications including other antihypertensive agents (additive blood pressure lowering effects), diuretics (increased risk of hypotension), lithium (increased lithium concentrations), NSAIDs (may reduce antihypertensive effect and worsen renal function), potassium-sparing diuretics or potassium supplements (increased risk of hyperkalemia), and digoxin (increased digoxin levels). Concurrent use with ACE inhibitors may increase the risk of renal impairment, hyperkalemia, and hypotension. Rifampin may decrease telmisartan concentrations. Always inform your healthcare provider about all medications, including prescription drugs, over-the-counter medications, vitamins, and herbal supplements.
Missed dose
If a dose of Micardis is missed, it should be taken as soon as remembered on the same day. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not take a double dose to make up for a missed dose. Maintaining a consistent dosing schedule is important for optimal blood pressure control. Patients may consider using pill organizers or setting daily reminders to help maintain regular dosing. If multiple doses are missed, contact your healthcare provider for guidance as blood pressure may not be adequately controlled.
Overdose
Symptoms of Micardis overdose may include hypotension, dizziness, tachycardia, bradycardia, and renal failure. In case of suspected overdose, seek immediate medical attention. Management should focus on supportive care, including close monitoring of vital signs and electrolyte balance. Treatment may include intravenous normal saline for hypotension and appropriate measures to correct electrolyte imbalances. Telmisartan is not significantly removed by hemodialysis due to high protein binding. The most likely manifestation of overdose would be hypotension and dizziness. Symptomatic treatment should be administered as clinically indicated.
Storage
Store Micardis tablets at room temperature between 15°C to 30°C (59°F to 86°F). Keep in the original container with the lid tightly closed to protect from moisture and light. Do not store in bathroom cabinets where humidity levels may fluctuate. Keep out of reach of children and pets. Do not use tablets that are discolored or show signs of deterioration. Properly discard any unused medication after the expiration date. Do not flush medications down the toilet or pour them into drainage unless instructed to do so. Consult your pharmacist or local waste disposal company for proper disposal methods.

Reviews
Clinical studies have demonstrated Micardis’ efficacy in blood pressure management with once-daily dosing providing 24-hour coverage. The ONTARGET trial showed telmisartan’s non-inferiority to ramipril in reducing cardiovascular events in high-risk patients. Many patients report satisfactory blood pressure control with minimal side effects. Healthcare professionals appreciate its metabolic neutral profile and potential benefits beyond blood pressure lowering. Some patients note the convenience of single daily dosing and generally good tolerability. However, individual responses may vary, and some patients may require combination therapy or alternative treatments. Long-term adherence rates are generally favorable compared to other antihypertensive classes.
