Common use
Dutanol 0.1% ointment is indicated for the short-term treatment of inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses. This includes, but is not limited to, plaque psoriasis (especially on elbows, knees, and scalp), atopic dermatitis, allergic contact dermatitis, nummular eczema, and lichen simplex chronicus. It is intended for use on adults and children over 12 years of age. Its use should be reserved for areas that require a potent corticosteroid and for conditions that have not responded adequately to milder topical preparations.
Dosage and direction
Apply a thin film of Dutanol to the affected area(s) twice daily, typically in the morning and evening. Gently rub in until absorbed. The amount needed depends on the size of the affected area; use the smallest amount necessary to lightly cover the skin. Treatment should be limited to a maximum of 14 consecutive days for plaque psoriasis and 7 days for eczematous dermatoses, unless directed otherwise by a physician. The total weekly dosage should not exceed 50g. Do not use under occlusive dressings unless explicitly instructed by a healthcare provider, as this significantly increases percutaneous absorption and the risk of adverse effects. Wash hands thoroughly after application, unless the hands are the treated area.
Precautions
For external use only. Avoid contact with eyes, mucous membranes, and open wounds. Do not use on the face, groin, or axillae (armpits) unless specifically prescribed by a doctor for those areas, as these regions are more susceptible to skin atrophy and other local adverse effects. Use in children should be undertaken with great caution and under close medical supervision due to their higher skin surface area to body mass ratio, which increases the potential for systemic absorption. If irritation or sensitization develops, discontinue use and consult a physician. Long-term continuous use should be avoided. Patients should be advised that this is a treatment for controlling symptoms, not a cure for the underlying dermatological condition.
Contraindications
Dutanol is contraindicated in patients with a known hypersensitivity to Dutanocort or any of the excipients in the formulation. Its use is absolutely contraindicated in the presence of viral skin infections (e.g., herpes simplex, varicella), fungal infections, and cutaneous tuberculosis. It must not be used to treat acne rosacea, perioral dermatitis, or untreated bacterial infections. Use is also contraindicated in patients with widespread plaque psoriasis where systemic therapy is more appropriate, due to the high risk of significant systemic absorption.
Possible side effect
The most common side effects are local and include burning, stinging, itching, irritation, and dryness at the application site. These are usually mild and transient. With prolonged use, the following may occur: skin atrophy (thinning), striae (stretch marks), telangiectasia (visible blood vessels), hypopigmentation, and folliculitis. Secondary infections may occur. Systemic absorption can lead to reversible hypothalamic-pituitary-adrenal (HPA) axis suppression, manifestations of Cushing’s syndrome, hyperglycemia, and glucosuria, particularly with prolonged use on large surface areas, use under occlusion, or use in young children.
Drug interaction
Formal drug interaction studies have not been conducted with topical Dutanol. However, systemic absorption of corticosteroids can potentially interact with other drugs. Caution should be exercised when using Dutanol concomitantly with other corticosteroid-containing products, systemic or topical, as this may increase the total corticosteroid burden and the risk of HPA axis suppression. No known interactions with systemic medications have been established for topical application when used as directed.
Missed dose
If a dose is missed, apply it as soon as remembered. However, if it is almost time for the next scheduled dose, skip the missed dose and resume the usual dosing schedule. Do not apply a double dose to make up for a missed one.
Overdose
Topically applied Dutanol can be absorbed in sufficient amounts to produce systemic effects. Acute overdose is unlikely from topical application. However, chronic overdose or misuse (e.g., application to large areas for prolonged periods, use under occlusion) can lead to symptomatic HPA axis suppression and corticosteroid insufficiency. In cases of suspected chronic overdose, treatment should be stopped gradually. There is no specific antidote. Management involves supportive care and may require systemic corticosteroid replacement therapy until HPA axis function recovers.
Storage
Store at room temperature between 15°C and 30°C (59°F and 86°F). Do not freeze. Keep the tube tightly closed when not in use to protect the contents from moisture and contamination. Keep out of direct sunlight. Keep out of reach and sight of children. Do not use after the expiration date printed on the tube and carton.
