Common use

Clarinex is primarily indicated for the relief of nasal and non-nasal symptoms of allergic rhinitis (seasonal and perennial) in adults and children 6 months and older. It is also approved for the treatment of uncomplicated skin manifestations of chronic idiopathic urticaria in patients 6 months and older. Healthcare providers may prescribe it off-label for other histamine-mediated conditions based on clinical judgment.

Dosage and direction

Adults and adolescents (12+ years): 5 mg orally once daily
Children (6–11 years): 2.5 mg (½ tablet or 2.5 mL syrup) once daily
Children (12 months–5 years): 1.25 mg (¼ tablet or 1.25 mL syrup) once daily
Infants (6–11 months): 1 mg (2 mL syrup) once daily

Tablets should be swallowed whole with water. Orally disintegrating tablets must be placed on the tongue immediately after opening the blister. May be taken with or without food. Dosage adjustment may be necessary in hepatic impairment.

Precautions

Use with caution in patients with renal impairment (CrCl <30 mL/min) or hepatic impairment. While Clarinex demonstrates minimal sedative effects, patients should exercise caution when engaging in activities requiring mental alertness until individual response is established. Pregnancy Category C: use only if potential benefit justifies potential risk to fetus. Excretion in human breast milk is probable—consider alternative feeding options during therapy.

Contraindications

Hypersensitivity to desloratadine, loratadine, or any component of the formulation. Contraindicated in patients with severe renal impairment (not on dialysis) or severe hepatic impairment without dosage adjustment. Orally disintegrating tablets contain phenylalanine and are contraindicated in patients with phenylketonuria.

Theoretical interactions with CNS depressants (alcohol, barbiturates, benzodiazepines) may potentiate sedation. Use caution with P-glycoprotein inhibitors. No meaningful interactions with warfarin observed in clinical studies.

Missed dose

If a dose is missed, take it as soon as remembered. If it is near the time of the next dose, skip the missed dose and resume regular dosing schedule. Do not double the dose to catch up.

Overdose

Symptoms may include drowsiness, tachycardia, and headache. There is no specific antidote. Gastric lavage may be considered if administered soon after ingestion. Provide supportive therapy including vital sign monitoring. Hemodialysis is unlikely to be beneficial due to high protein binding.

Storage

Store at 20–25°C (68–77°F). Excursions permitted to 15–30°C (59–86°F). Keep blister packages sealed until use. Protect from moisture. Store syrup in original container at room temperature—do not freeze. Keep all medications out of reach of children.
