Common use

Ciprodex is primarily prescribed for bacterial conjunctivitis caused by susceptible strains of microorganisms, including Staphylococcus aureus, Streptococcus pneumoniae, Haemophilus influenzae, and Pseudomonas aeruginosa. It is also indicated for the treatment of corneal ulcers with associated inflammation. Ophthalmologists may employ it perioperatively in ocular surgery prophylaxis or for managing post-operative inflammation and infection risk. Off-label uses may include treatment of blepharitis or dacryocystitis when inflammation is a significant component.

Dosage and direction

Shake the bottle well for 5 seconds before each use. Instill 1–2 drops into the affected eye(s) every 12 hours for 7–10 days, or as directed by your ophthalmologist. For corneal ulcers, treatment may be initiated with more frequent dosing (every 2 hours while awake) for the first 48 hours, followed by the standard twice-daily regimen. Tilt the head back, pull down the lower eyelid to create a pouch, and instill drops without touching the dropper tip to any surface. Wait at least 5 minutes before instilling any other eye medications.

Precautions

Do not wear contact lenses during treatment. Patients should discontinue use immediately and consult their physician if signs of superinfection (including fungal infection) appear. Use with caution in patients with corneal perforation or thinning, as corticosteroids may delay healing. Prolonged use may result in ocular hypertension, glaucoma, optic nerve damage, or cataract formation. Monitor intraocular pressure regularly during extended therapy. Not for injection or otic use—ocular formulation only.

Contraindications

Hypersensitivity to ciprofloxacin, other quinolones, dexamethasone, or any component of the formulation. Contraindicated in viral infections of the cornea and conjunctiva (including herpes simplex), mycobacterial eye infections, and fungal diseases of ocular structures. Should not be used after uncomplicated removal of a corneal foreign body where infection is not present. Avoid in patients with a history of steroid-induced intraocular pressure elevation.

Possible side effect

Common: transient burning/stinging (15–20%), conjunctival hyperemia, blurred vision, foreign body sensation, itching, white crystalline precipitate (resolves upon discontinuation). Less common: corneal deposits, lid edema, tearing, photophobia, allergic reactions. Rare: corneal perforation (in susceptible eyes), secondary infection, punctate keratitis, increased intraocular pressure, optic nerve damage. Systemic absorption is minimal but may theoretically cause hypersensitivity reactions in susceptible individuals.

Drug interaction

Although topical administration minimizes systemic interactions, theoretical interactions exist. Concomitant use with other ocular corticosteroids may potentiate intraocular pressure elevation. Other ophthalmic solutions should be administered at least 5 minutes apart to avoid dilution or incompatibility. Oral anticoagulants—monitor for potentiation of effect due to possible steroid interaction. Avoid concurrent use with other quinolones systemically due to additive effects. Not studied with ocular inserts or punctal plugs.

Missed dose

Apply the missed dose as soon as remembered, unless it is nearly time for the next scheduled dose. Do not double the dose to catch up. Maintain the regular dosing schedule. If multiple doses are missed, contact your healthcare provider for guidance. Consistency in administration is important for clinical efficacy and preventing antibiotic resistance.

Overdose

Topical overdose is unlikely to cause systemic effects due to minimal absorption. If accidentally ingested, symptomatic treatment is recommended. Ocular overdose may cause increased irritation, redness, or blurred vision—rinse eye thoroughly with sterile saline or warm water. In case of accidental oral ingestion, contact poison control center. No specific antidote exists; provide supportive care based on symptoms.

Storage

Store at 2°C–25°C (36°F–77°F). Do not freeze. Keep bottle tightly closed when not in use. Discard 28 days after opening—write opening date on bottle label. Protect from light and excessive heat. Keep out of reach of children and pets. Do not use if solution changes color or becomes cloudy.
