Besivance (besifloxacin ophthalmic suspension) 0.6% is a prescription topical fluoroquinolone antimicrobial indicated for the treatment of bacterial conjunctivitis. It is specifically formulated to target a broad spectrum of ocular pathogens with potent bactericidal activity, offering both high efficacy and a favorable safety profile. Its unique suspension base enhances corneal contact time and patient comfort, making it a first-line choice for eye care professionals managing bacterial eye infections.

Features

  • Contains besifloxacin hydrochloride equivalent to 0.6% besifloxacin
  • Preservative-free formulation in a multidose bottle
  • Broad-spectrum activity against Gram-positive, Gram-negative, and atypical bacteria
  • Designed as a sterile, isotonic ophthalmic suspension with mucoadhesive properties
  • Durable polymer-based vehicle for prolonged ocular surface residence

Benefits

  • Rapid and effective eradication of common bacterial pathogens causing conjunctivitis
  • Low potential for developing bacterial resistance due to its dual-target mechanism
  • Reduced risk of irritation or allergic response thanks to preservative-free composition
  • Enhanced bioavailability and sustained drug release at the infection site
  • Convenient twice-daily dosing supports patient adherence and treatment success
  • Clinically proven to achieve high microbial eradication and clinical resolution rates

Common use

Besivance is primarily used for the treatment of bacterial conjunctivitis in patients 1 year of age and older. It is prescribed when signs and symptoms such as purulent discharge, conjunctival injection, eyelid edema, and ocular discomfort suggest a bacterial etiology. Clinicians may also consider its use in cases where contact lens wear, trauma, or postoperative care increases the risk of bacterial ocular surface infection.

Dosage and direction

Instill one drop into the affected eye(s) twice daily, approximately 8 to 12 hours apart, for 7 days. Wash hands before use. To avoid contamination, do not touch the dropper tip to any surface. If using other topical ophthalmic products, administer them at least 5 minutes apart. Shake the bottle well before each use.

Precautions

  • Not for injection or use in the ear
  • Contact lenses should not be worn during the course of treatment
  • If symptoms worsen or do not improve within a week, reevaluation is recommended
  • Use with caution in patients with a history of hypersensitivity to fluoroquinolones
  • Should be discontinued at the first sign of rash or allergic reaction

Contraindications

Besivance is contraindicated in patients with a known hypersensitivity to besifloxacin, other quinolones, or any component of the formulation.

Possible side effects

The most commonly reported adverse reactions (occurring in 1–5% of patients) include:

  • Blurred vision
  • Eye irritation
  • Eye pain
  • Eye redness
  • Headache

Rare but serious side effects may include corneal deposits, superinfection, or allergic reactions.

Drug interaction

Although systemic absorption is low, caution is advised when using Besivance concomitantly with other fluoroquinolone-class drugs. No specific pharmacokinetic interactions have been identified in clinical studies.

Missed dose

If a dose is missed, it should be administered as soon as possible. However, if it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.

Overdose

Ocular overdose may be rinsed gently with warm water. Systemic overdose is unlikely due to minimal absorption, but symptomatic treatment is recommended if accidental ingestion occurs.

Storage

Store at 2°C to 25°C (36°F to 77°F). Do not freeze. Keep the bottle tightly closed when not in use. Discard the bottle 14 days after opening.
