Common use

Dosage and direction
Adults:
- Respiratory/skin infections: 500 mg on day 1, followed by 250 mg once daily on days 2-5
- Sexually transmitted infections: Single 1 g dose
- COPD exacerbations: 500 mg daily for 3 days
Pediatric dosing:
- Based on weight for otitis media and community-acquired pneumonia
- Typically 10 mg/kg on day 1 (maximum 500 mg), then 5 mg/kg on days 2-5
Take exactly as prescribed, with or without food. Complete the full course even if symptoms improve.
Precautions
- Use with caution in patients with hepatic impairment
- Monitor for development of Clostridium difficile-associated diarrhea
- May prolong QT interval; use caution in patients with cardiac conditions
- Not recommended for patients with known QT prolongation or arrhythmias
- Use during pregnancy only if clearly needed (Category B)
Contraindications
- Known hypersensitivity to azithromycin, erythromycin, or other macrolides
- History of cholestatic jaundice/hepatic dysfunction with previous azithromycin use
- Concurrent use with ergot derivatives or pimozide
- Patients with known QT prolongation or ventricular arrhythmias

Drug interaction
- Antacids containing aluminum or magnesium: Separate administration by 2 hours
- Warfarin: Monitor INR closely
- Digoxin: May increase digoxin concentrations
- Cyclosporine: Monitor cyclosporine levels
- Nelfinavir: Increases azithromycin levels
- Other QT-prolonging agents: Increased risk of cardiac arrhythmias
Missed dose
Take the missed dose as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume regular schedule. Do not double doses to make up for a missed dose.
Overdose
Symptoms may include severe nausea, vomiting, diarrhea, and temporary hearing loss. Management is supportive; gastric lavage may be considered if presented soon after ingestion. There is no specific antidote.
Storage
- Store tablets at 15-30°C (59-86°F)
- Keep oral suspension at room temperature; discard after 10 days
- Keep in original container, tightly closed
- Protect from excessive moisture and light
- Keep out of reach of children
