Common use
Asthalin is commonly prescribed for the management of bronchospasm in conditions such as asthma, exercise-induced bronchoconstriction (EIB), and chronic obstructive pulmonary disease (COPD), including emphysema and chronic bronchitis. It is also used prophylactically to prevent bronchospasm triggered by allergens, exercise, or environmental factors. In clinical settings, it may be administered via nebulization for patients experiencing severe respiratory distress.
Dosage and direction
Dosage must be individualized based on patient need, severity of condition, and delivery device. For inhalation via pMDI (100 mcg/puff): adults and children over 4 years—1–2 puffs every 4–6 hours as needed. Maximum 8 puffs in 24 hours. For nebulization: 2.5 mg to 5 mg up to 4 times daily. Oral syrup: adults—2–4 mg (4–8 ml) 3–4 times daily; children—0.1 mg/kg/dose 3 times daily (not to exceed 2 mg 3–4 times daily). Shake the inhaler well before use. Prime the inhaler if new or unused for 2 weeks. Administer during slow, deep inhalation; hold breath for 10 seconds if possible. Rinse mouth after use to reduce systemic absorption and potential side effects.
Precautions
Use with caution in patients with cardiovascular disorders (e.g., hypertension, arrhythmias, coronary insufficiency), diabetes mellitus, hyperthyroidism, or convulsive disorders. Monitor for paradoxical bronchospasm—discontinue immediately if it occurs. Excessive use may lead to tolerance and loss of effectiveness; increased use may indicate deteriorating asthma control. Not intended for regular daily use unless specifically directed. Pregnancy Category C: use only if potential benefit justifies potential risk to the fetus. Contraindications

Possible side effects
Common side effects include tremor, nervousness, headache, tachycardia, palpitations, muscle cramps, and dry or irritated throat. Less frequently, patients may experience hypokalemia, increased blood pressure, dizziness, nausea, and sweating. Paradoxical bronchospasm, angina, and sleep disturbances have been reported rarely. Allergic reactions such as rash, urticaria, and oropharyngeal edema are possible.
Drug interaction
Concomitant use with other sympathomimetic agents may potentiate cardiovascular effects. Use with monoamine oxidase inhibitors (MAOIs) or tricyclic antidepressants may enhance cardiovascular effects. Beta-blockers (non-selective) may antagonize bronchodilating effects and induce severe bronchospasm in asthmatic patients. Diuretics or corticosteroids may enhance hypokalemic effect. Use cautiously with xanthine derivatives, steroids, or diuretics.
Missed dose
Asthalin is typically used on an as-needed basis for symptom relief. If using a scheduled dosing regimen and a dose is missed, administer it as soon as remembered unless it is nearly time for the next dose. Do not double the dose.
Overdose
Overdose may lead to exaggerated adrenergic effects such as angina, hypertension, hypokalemia, seizures, tachycardia, and cardiac arrest. Symptoms may also include nausea, vomiting, and metabolic acidosis. Treatment is supportive and symptomatic; cardioselective beta-blockers may be considered but with caution due to risk of bronchospasm.
Storage
Store at controlled room temperature (15–30°C). Keep the canister away from direct heat or sunlight. Do not puncture, incinerate, or expose to temperatures above 50°C. Keep out of reach of children. For nebulizer solution: store in foil pouch until use; protect from light.
