Common use
Cialis is primarily prescribed for the treatment of erectile dysfunction (ED) in adult men. It is also indicated for the treatment of the signs and symptoms of benign prostatic hyperplasia (BPH) in adult men. For BPH, it is used to improve urinary flow and reduce symptoms such as hesitancy, weak stream, and nocturia. In some cases, it may be prescribed for both conditions concurrently. It is not indicated for use by women or children.
Dosage and direction
Dosage must be individualized based on the patient’s needs, tolerability, and the specific indication.
- For Erectile Dysfunction (On-demand): The recommended starting dose is 10 mg taken prior to anticipated sexual activity. Depending on efficacy and tolerability, the dose may be increased to 20 mg or decreased to 5 mg. The tablet should be taken at least 30 minutes before sexual activity. The maximum recommended dosing frequency is once per day.
- For Erectile Dysfunction (Once-daily): The recommended dose is 2.5 mg taken approximately at the same time every day, without regard to timing of sexual activity. This regimen is appropriate for men who anticipate sexual activity more than twice weekly. The dose may be increased to 5 mg based on individual response and tolerability.
- For Benign Prostatic Hyperplasia (BPH): The recommended dose is 5 mg taken once daily at approximately the same time each day.
- For both ED and BPH: The recommended dose is 5 mg taken once daily.
The tablet should be swallowed whole with a glass of water. It can be taken with or without food. Do not break, split, or crush the tablet.
Precautions
- Cardiovascular Status: Patients should be assessed for cardiovascular suitability for sexual activity prior to initiating treatment. PDE5 inhibitors, including Cialis, have systemic vasodilatory properties and can cause transient decreases in blood pressure.
- Prolonged Erection: Patients who experience an erection lasting more than 4 hours (priapism) should seek immediate medical attention. If not treated promptly, priapism can lead to permanent damage to penile tissues and long-term erectile dysfunction.
- Sudden Vision Loss: Non-arteritic anterior ischemic optic neuropathy (NAION) has been reported rarely post-marketing. It is not possible to determine if these events are related directly to PDE5 inhibitors or to other factors. Patients who experience a sudden loss of vision in one or both eyes should discontinue Cialis and seek immediate medical attention.
- Sudden Hearing Loss: Cases of sudden decrease or loss of hearing have been reported. Patients who experience these symptoms, sometimes accompanied by tinnitus and dizziness, should discontinue Cialis and seek prompt medical attention.
- Alpha-blockers & Antihypertensives: Concomitant use can potentiate the blood-pressure-lowering effects. A specific time separation between doses is not effective due to Cialis’s long half-life. Concomitant use is not recommended.
- Renal or Hepatic Impairment: Dosage adjustments are recommended for patients with moderate to severe renal impairment or mild to severe hepatic impairment. Use is not recommended in patients with severe hepatic impairment.
Contraindications
Cialis is contraindicated in patients with:
- A known hypersensitivity to tadalafil or any component of the tablet.
- Concurrent use of any form of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate or dinitrate) or nitric oxide donors (e.g., amyl nitrite “poppers”) in any form, either regularly or intermittently. The concomitant administration can cause a profound and life-threatening drop in blood pressure.
- Concurrent use of guanylate cyclase stimulators, such as riociguat.
Possible side effect
The most common side effects of Cialis are generally mild to moderate and often diminish with continued use. They are a result of the drug’s vasodilatory effects.
- Very Common (≥1/10): Headache, dyspepsia (indigestion).
- Common (≥1/100 to <1/10): Back pain, myalgia (muscle ache), nasal congestion, flushing, pain in limb, dizziness.
- Uncommon (≥1/1,000 to <1/100): Gastroesophageal reflux disease (GERD), nausea, vomiting, rash, blurred vision, eye pain, eyelid swelling, hypotension, palpitations, tachycardia, chest pain, transient global amnesia.
- Rare: Priapism, NAION, hearing loss, serious cardiovascular events (e.g., myocardial infarction, stroke) have been reported in temporal association with sexual activity and PDE5 inhibitor use.
Drug interaction
Cialis is primarily metabolized by CYP3A4. Concomitant use with drugs that affect this enzyme system can alter its plasma concentrations.
- Nitrates: Absolute Contraindication. Concomitant use can cause severe hypotension.
- Alpha-blockers (e.g., doxazosin, tamsulosin): Not Recommended. Significant potentiation of blood-pressure-lowering effects.
- Potent CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole, ritonavir): For patients on daily Cialis, the dose should not exceed 2.5 mg. For on-demand dosing, consider a maximum dose of 10 mg no more than once every 72 hours. Avoid use with ketoconazole or itraconazole 400 mg daily.
- Potent CYP3A4 Inducers (e.g., rifampin, phenobarbital, phenytoin, carbamazepine): May significantly decrease tadalafil exposure, potentially reducing efficacy. Consider alternative therapy or a higher starting dose (20 mg) with careful monitoring.
- Antihypertensives: May potentiate the blood-pressure-lowering effect. Monitor blood pressure.
- Alcohol: Excessive consumption (≥5 units) in combination with Cialis may increase the potential for orthostatic signs and symptoms, including increased heart rate, decreased standing blood pressure, dizziness, and headache.
Missed dose
- For the on-demand (10 mg or 20 mg) regimen: This regimen is not scheduled. Take a dose approximately 30 minutes before anticipated sexual activity. Do not take more than one dose in a 24-hour period.
- For the once-daily (2.5 mg or 5 mg) regimen: If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and take the next dose at the regular time. Do not take a double dose to make up for a missed one.
Overdose
In cases of overdose, standard supportive measures should be adopted as required. Hemodialysis contributes negligibly to tadalafil elimination. The primary expected effects of overdose would be an increase in the severity of dose-related adverse reactions such as headache, flushing, dyspepsia, back pain, and significant hypotension. Management of hypotension should include cardiovascular support with elevation of the legs and, if necessary, intravenous fluids. If required, a vasopressor (e.g., phenylephrine) may be considered, but not epinephrine.
Storage
- Store Cialis at room temperature between 20°C to 25°C (68°F to 77°F).
- Keep the tablets in their original blister package or bottle to protect from light and moisture.
- Keep all medications out of the reach of children and pets.
- Do not use after the expiration date printed on the packaging.
